• Photocopy of at least one valid ID (front and back) with picture: Driver's License, Passport, Voter's ID, SSS/GSIS ID, BIR/TIN ID, PRC ID, Company ID issued by private entities or institutions registered by BSP, SEC, or IC.
• Proof of Income or a copy of your credit card statement from another bank.
The annual membership fee is free on the first year and Php 4,000 on the second year and onwards.

a. Credit card type
b. Credit card number
c. Transaction date and amount
