1. One (1) photo-bearing and unexpired identification documents (front and back copy):

• Identification documents issued by the Government of the Republic of the Philippines including its political subdivisions, agencies and instrumentalities (e.g., PhilID, ePhilID, National ID, Passport, Driver’s License, etc.)
• Company ID issued by private entities or institutions registered in BSP, SEC or IC

2. Proof of income or copy of your credit card statement from another bank.

• Principal Cardholders:
  • First year: Annual fee waived
  • Succeeding years: Php 2,500
• Supplementary Cardholders:
  • First supplementary card: Annual fee waived for life
  • 2nd to 6th supplementary cards: Php 1,250
