1. The available payout options for Planholders/claimants are:
a. Automatic credit to Planholder's personal peso account. (Crediting to another individual’s account is not allowed.)
b. Cash pickup at any BPI branch. A reference number will be sent to the mobile number upon approval of claim.
2. Ayala Plans reserves the right to request additional documents as may be needed to process the request especially for Planholders or claimants with discrepancy/ies in their personal information versus the records with Ayala Plans or with the Bank. (Inform us ahead should there be a need to update your records such as change of name from maiden to married, correction of date of birth, etc.)
3. A re-processing fee of Php 200.00 (subject to change) will be required for clients who fail to claim their proceeds at the branch 90 calendar days after approval and will re-file for claim at a later date. (Applicable to those who chose ‘cash pickup’ as payout option.)

b. Requirements for claim with less than Php 250,000.00 amount (excluding those abroad, those that cannot physically sign and for death claim and other exception cases as determined by Ayala Plans).

If Nominee is still a minor and Guardian is the Natural Parent (For deceased Planholder with an Education Plan)
1. Original PSA/NSO or original Certified True Copy issued by the Local Civil Registrar of the following:
a. Death Certificate of the Planholder
b. Marriage Certificate of the Planholder
c. Birth Certificate of Nominee
2. Clear copy of one (1) valid ID with picture, signature, and date of birth (government-issued ID with three (3) specimen/original signatures on the page) of:
a. Planholder
b. Nominee
c. Guardian
3. Original Signed/Accomplished Plan Benefit Pay-out form (accomplished and signed by the Guardian)
4. Original Notarized Affidavit of Guardianship
5. Clear proof of account if chosen payout option is credit to a peso bank account of the guardian which shows bank name, and account number under the name of the claimant.
6. Original Plan Contract and Certificate of Full Payment (for maturity claim)
For Nominee of age (For deceased Planholder with an Education Plan)
1. Original PSA/NSO or original Certified True Copy issued by the Local Civil Registrar of the following:
a. Death Certificate of the Planholder
b. Birth Certificate of Nominee
c. Marriage Certificate of married Nominee only
2. Clear copy of one (1) valid ID with picture, signature, and date of birth (government-issued ID) with three (3) specimen/original signatures on the page.

c. Plan Benefit Payout Form
4. Clear proof of account if chosen payout option is credit to a peso bank account which shows bank name, and account number under the name of the Nominee.
5. Original Plan Contract and Certificate of Full Payment (for maturity claim)
Payout Re-Processing (previously processed and unclaimed benefit)
1. Original Signed/Accomplished Plan Benefit Pay-out form
2. Clear copy of one (1) valid ID with picture, signature, and date of birth (government-issued ID) with three (3) specimen/original signatures on the page.
3. Clear copy of deposit slip as proof of payment (re-processing fee)- send us an email for the Bank Reference Number (BRN). Pay at any BPI branch:
